I’ve been positive for 7 years, should I start medication?

I tested positive in 2012 with my CD 4 count being about 800…..i am not on antiretroviral drugs yet and i haven’t fallen sick for a day….can i start ARVs

Though you may be feeling well, you still have HIV. If you want to control your HIV you’ll need to be on medication. So yes, starting medication is the right thing to do. Please see here: https://i-base.info/guides/starting
If you’ve not had a CD4 count test done since 2012, you’ll need to have one done. Though you’ve not been sick its very likely that it will have dropped. However, as to how much its not possible to say.
