New guidelines on the management of persistent low-level viraemia (LLV) by an international panel of more than 100 experts and are now published in Lancet HIV. Importantly, they recognise there can be two distinct mechanisms for viral rebound: (i) ongoing replication which can be resuppressed or (ii) viral production from reservoir site which is non-suppressible.
This report from the 6th BHIVA/BASHH conference cover four talks given to a packed lunchtime workshop. This included data from Kings College Hospital in South London the 50% of Chemsex related A&E admissions in people living with HIV had disconnected from HIV care and that median viral load was 250,000 c/mL.
New UKHSA resource continues to support using the 4CMenB vaccine despite failure of the Australian GoGoVax RCT to show any benefit – and despite BHIVA doctors recommending caution.
This latest report based on national data rather than just from PEPFAR-funded clinics shows that decades of progress are steadily being drawn backwards. HIV testing, PrEP, CD4 and new ART all significantly reduced with especially harsh results for children.
Top-line results from the phase 3 ISLEND-1 and -2 studies might enable FDA submission in the US, but the EMA and MHRA might demand 96-week data before submission.
