The Yellow Card Scheme is vital in helping the Medicines and Healthcare products Regulatory Agency (part of the Department of Health) monitor the safety of the medicines that are on the market.
If you think a medicine or herbal remedy has caused an unwanted side effect, you can report the problem on a Yellow Card.
You can report a suspected side effect in the following ways:
- On the Yellow Card website.
- On a Yellow Card form, which you can fiĀnd at pharmacies, GP surgeries or from the Yellow Card hotline by calling freephone 0808 100 3352 during business hours. The Yellow Card form can also be downloaded, to print and complete.
