Will my GP disclose my status to my work?

Hi,
I have recently been offered a new job and with this offer I must fill out a health check questionnaire. I do not want to disclose my status on this but will they contact my GP and be told by him? I am now very worried about this.

You do not have to disclose your status. You should talk to your GP and tell him that you do not want them to know you have HIV. He can only disclose if he has your permission to do so.
The only exceptions to this rule are those organizations who carry out full medical check ups and test for HIV such as the armed forces.
