How do I take my meds after operation?

I’m having a routine procedure just after Christmas and it will involve being nil by mouth for almost 24 hours. I take my Truvada and darunavir/r regularly at 8am. The day before the procedure I will be NBM from 4pm and won’t be able to have something to eat again until 4pm the next day at the earliest.

Is it best to be a whole 8 hours late on one day, or should I stagger the doses leading up to it by taking them at say 10am, then noon then 2pm and 4pm before returning to 8am the next day. Or will a slight overdose reducing the interval to 16 hours the two days before the procedure do we any harm?

My VL is undetectable.

It’s a good idea to ask your doctor about this, as you may be able to still take your meds on an empty stomach the day of the procedure.

If you can’t take the morning dose the easiest thing may be to miss your dose that day. The risk of developing resistance from one missed dose is very low. Because your viral load is undetectable this is even smaller.

The patient information leaflet for darunavir/r suggests that if you are more than 6 hours late with a dose you should wait for the next dose. With Truvada the advice is to take the dose if you are not later than 12 hours.  This is because of the different drug levels of these meds, and the risk of side effects.
