Shall I start treatment?

Just wanted to ask if you think I should start treatment – been diagnosed positive for 12 months; had a negative test 2 years ago. My initial CD4 was 360 and VL 400. Three months later CD4 480 and VL 600. Three months later CD4 460 and VL 600. Three months later CD4 416 VL 400 and this week CD4 460 and VL 400. My percentage has averaged around 30%.

Would value your opinion.

Answer

Unless you have any clinical condition that will improve because of starting treatment, there is no reason to start now. The majority of the guidelines currently recommend a treatment start when the CD4 count is near 350. In your case it is way above. In addition, your VL is really low and, apparently, your body succeeds to suppress successfully the virus. On top of that, your percentage is great.
