If you’re just been diagnosed and with a CD4 of 78 you’re going to need to start medication. Please see here for more info:

This is something that your healthcare provider should talk through with you. Due to your CD4 count being below 200, you should also take the following septrin, also known as co-trimoxazole. It is a double antibiotic that is active against many infections, especially in people who have a low CD4 count.
