Q and A

Question

Do I need to extend PEP after being exposed midway through?

Hello Here are the sequence of events need guidance:

1. April 25th my condom slipped when I pulled out since I panicked, concerned my Doctor started me on PEP Truvada and Tivicay which was started in time (40 hours) on April 27th.
2. I have been on PEP for two good weeks and had a very similar incident, where the condom slipped again. I don’t know the person’s status is unknown. May 12th, this is quite a bizarre instance of occurring twice. I think I switched to a new condom brand.

I have two clarifications here:

1. Since I am on PEP and one of the medication Truvada is also used as PREP what are the risk here for me with given incident?
2. I am still taking PEP and have 15 more days of dosage left which will end on May 28th. So given that I am already on. The meds and the risk exposure of the issue should I just complete rest of my course and extend the PEP additional 2 weeks?

NOTE: I have been very careful and these unfortunate incidents happened twice.

Appreciate your guidance

Answer

Hi, how are you doing?

You do not need to extend your course of PEP. This is because, as you have mentioned your regimen of PEP contains the drug combination used in PrEP. For this reason PEP will have been acting as PrEP in your body and prevent further risky exposures while midway through the course.

You can finish your course of PEP when originally expected (when you get to the end of the next 15 days).

I understand that you have said this is a rare occurrence but have you considered PrEP? This would prevent any further panicking if you have any future problems with condoms.

Hi Dany, as you are already on PEP you do not need to extend/change how long you are taking PEP. While on PEP you are protected from further exposures. You can compelte your course when originally planned. Please see here for more information: https://i-base.info/qa/factsheets/pep-faq

Hi Nono, do you mean PEP or PrEP? Do you know the name of the treatment you are taking? and is your partner on treatment?

    Hello.
    My partner is positive.
I started Prep after but within 24hrs.

Hi Nono, as you had already been on PEP this will have provided some protection. Does this mean you took your last dose of PEP. Had a potential exposure on this same day, and then the day after you started PrEP?

If this is the case the following doses of PrEP will have provided protection by following the ‘on-demand’ regimen for PrEP. This means taking PrEP the following 2 days after an exposure when having already had PrEP/PEP the day of/before.

    Hello.
I had a second exposure on day 28. That day I took my last pill and had an exposure again the following day started Prep.
    I am worried

Hi Nono, when did you get exposed after finishing the course of PEP? Did you start PrEP before or after the exposure?

  8. Nono

    Hello.
I would to find out if you are on day 28 of finishing PEP then get exposed again but start Prep the next day what are the chances?
The test before Prep was negative.

Hi Jhon, no this is not a risk for HIV transmission. For more information please see here: https://i-base.info/qa/factsheets/hiv-transmission-and-testing

Good day. I have a doubt and I wonder if you could help me answer it. I don’t know if I have a real exposure to hiv or not. Some sites tell me it is other tell me it is not. So I was with this guy who was sucking me off and in that moment we both were really turn on, so then he stands up and shows me his ass and tolds me to just rub and poke with the tip because he didnt came prepare for penetration. So I rub it and slightly poke his anus. But the tip didnt go in and now that im thinking more calmly Im full of doubts on whenever I was put at risk or not. Could you give me your opinion on the matter?.
