Q and A

Question

Can TLD be used as PEP?

Can TLD help someone who has been in contact with an HIV positive person to prevent them from contracting HIV before 72 hrs?

Answer

Hi, how are you?

Yes you can. TLD is a combination that can be used as PEP and for the management of active HIV infection. PEP stands for post exposure prophylaxis. PEP is used to prevent established infection after exposure.

TLD is a first choice recommendation for PEP. The course lasts 28 days following a high risk exposure. It is only used in cases where the exposure was within 72 hours. After this, regardless of what drug is prescribed, PEP will not work.

However, if your partner was using effective HIV treatment (ART) , you will not be at risk. Having an undetectable viral load on ART means HIV is not a risk. Please see:

    Hi,

I have protected sex with someone. Then a week later, he come clean that he is taking Acritega for 4 months.

There is no accident with the protection during the sex.

Should I be worried about getting transmitted? I am negative.

Hi Tee, TLD can be taken with doxycycline. There is no interaction between these drugs.

There is some interaction between aciclovir and tenofovir (TLD). As they are both removed from the body by the kidneys, there is some risk that the kidneys could become overwhelmed leading to an increase in both drugs in the body. This interaction has not been confirmed but because of how the drugs work you should speak with your doctor before starting to take them together.

Hi Mark, yes it is okay to take these other medications while you are on TLD. TLD are not the cause of your symptoms. TLD does not cause mood swings.

It is most likely that you full-day road trip are the cause of your symptoms as you have mentioned.

Hi! I had unprotected sex on 10th Sep (1 am) with someone who ‘might have’ HIV, and i using Acriptega LTD as a PEP on 11 Sep (2 pm). Now on 13th Sep, i have a fever, sore throat and dizziness. So my question is:

    1. Is the symptoms is effect of the LTD?
    2. Is it okay to use some other medicine like paracetamol and dexamethasone for my current symptoms?
    3. Do LTD cause any swing mood or emotions?

FYI: i also did a full-day road travel on 10th Sep, Is this what caused me sick? (fever, etc.)

Hi Kelvin, TLD is a suitable ARV combination to be used as PEP. TLD will be as effective as any other combination used as PEP.

Hi Thabo, while you are on PEP you are protected from further exposures. You do not need to do anything more. There is no risk of transmission from this event.

Please see here for more information about PEP: https://i-base.info/qa/factsheets/pep-faq

Hi Tumi, you do not need to be on PEP for your partners exposure. This is very low risk and it is not recommended to be on PEP in your case. Though it is recommended to either not have sex while your partner is not sure of their status or to use a condom.

PrEP is a slightly different medication to PEP. It contains fewer drugs. Acriptega will work to prevent HIV transmission and act as PrEP, however it is not recommended for this use. PrEP is most commonly a drug called Truvada: https://i-base.info/guides/prep
