Hi Libuseng, whether or not you start treatment, depends on talking with your doctor. Your CD4 count shows that the risk of HIV-related health problems is still low. Some countries recommend treatment for everyone who tests positive. Others recommend waiting until the CD4 count drops a little, until this is below 500. Part of the decision involves you choosing what is right for you.

The risk for transmission relates to your viral load. Your CD4 count is a rough guide to how well your immune system is doing.

If you have a CD4 count of much less than 600 you can still transmit HIV if you’re not on treatment and if you’re not undetectable.

You can find out more about viral load and risk here:

Can a person with CD4 count of 600 trasmit the virus to another person?

Hi Candle – thanks – it is good that you are finding out about infomration to help your father. He really needs to be on HIV meds so that his CD4 count can become stronger. Even though his count is now low, treatment can still work and he might still make a good recovery.

I don’t know much about HIV or aids, I found out last week that my father has aids. I probably still wouldn’t know if it weren’t for how sick he is right now. Today I was told his count is 24.. how bad is that??

Hi, A CD4 count of 650 is strong. Starting treatment with a high CD4 count is now recommended in many guidelines around the world.

The meds used in HIV treatment (ART) in 2016 are now safe and effective. HIV drugs (ARVs) in the past were sometimes associated with body shape changes and other side effects, but with the new ARVs these side effects are much less common.

Please see this Q&A on starting treatment with a high CD4 count.

My CD4 is 650. I just want to know if the treatment will not affect me in anyway? Like skin changes. Body shape changes etc, I am so worried.
