Can TLD be used as PEP?

Can TLD help someone who has been in contact with an HIV positive person to prevent them from contracting HIV before 72 hrs?

Yes you can. TLD is a combination that can be used as PEP and for the management of active HIV infection. PEP stands for post exposure prophylaxis. PEP is used to prevent established infection after exposure.

TLD is a first choice recommendation for PEP. The course lasts 28 days following a high risk exposure. It is only used in cases where the exposure was within 72 hours. After this, regardless of what drug is prescribed, PEP will not work.

However, if your partner was using effective HIV treatment (ART) , you will not be at risk. Having an undetectable viral load on ART means HIV is not a risk. Please see:

    Hi Francis, if this was a high risk exposure you can use PEP. This is a 28 day course of treatment that needs to be started within 72 hours of an exposure.

    PEP can be access from the emergency department and/or health clinic.

    Hie. I just got exposed to the hiv virus about 10hrs ago and I really do not know what to do or what to get so as to avoid infection. Please help with advice on what I need to take urgently and where I can get it?

    Hi Chan, sorry for missing this. A 3rd generation is only conclusive from 12 weeks after exposure.

    A negative test after 50 days is encouraging and more likely suggests that you are HIV negative but this will require confirming.

    Hi Chan again
    Got tested with tri dot hiv 1&2 antibodies only on 50th day after exposure, test was done in laboratory with vein blood. Came back as negative
    How accurate is it?

    Hi Chan, for RNA quantitative real time PCR tests are not used to confirm HIV status. You will need to use an antibody test.

    The earliest test you can use is a 4th generation antigen/antibody test that is conclusive from 6 weeks after the last dose of PEP.

    Hi,
    I got tested on 26th day after taking TLD as pep for 28 days so 54 days after exposure,the test i took was HIV-1 RNA Quantitative Real Time PCR and the result was negative or target not detected,
    How accurate is this test and is it a good thing i got negative in this test? Do i need to take another test for more accurate result? I heard it is like 98% accurate at this timing.

    Hi David, these are 2 drugs that can be used to make up PEP.

    These are often combined with dolutegravir to create a 3-drug combination. TDF and lamivudine alone are not suitable to be used as PEP.
